Hello! My name is Jen Jones. Welcome to the Eisenhower High School Health Room website. I hope that this page provides any information or health related forms that you may be looking for pertaining to your student.

I would like to start off by telling you that it is my goal to provide a safe and healthy environment for your kiddos while they are at Eisenhower HS. My background is in Emergency Room, Trauma and Pediatric medicine. I worked several years at Wesley Medical Center in the Emergency Department, Trauma and Pediatric Emergency as a nurse and charge nurse, and then decided that it was time for a change. My family and I reside in Conway Springs where we enjoy fishing, watching movies and just hanging out together around a bonfire.

Below you will find a few of our guidelines that I believe will provide the best information for you this year. Links to forms are provided also. Please remember that every new school year you will need to fill out the Annual Health Information form included in your online enrollment packet. This form is very important to alert me to ANY and ALL health concerns that you or your child may have.

If your child should receive any restrictions, medical clearances or Dr. notes throughout the year, please provide me with a copy for their file.
